I have too many to count.
Accidentally caused the release of a major demon on the City. My level 11 wizard character had Hold Person prepared and yet I rolled an opposed strength check to stop a lowish level barbarian from breaking the Orb that could banish the demon.
My level 2 Tychean sorc that literally no one had ever met before walked up to the Temple during Lunatic High's assault and was like HI I'm joining you guys. Insta-fugued by the adventurers laying siege to it. Deserved it.
Sat behind a certain fighter who brought Aldek to 3 HP during the Red Hart war healing said fighter instead of trying to damage a Near Death Aldek.

I once complained about level 9 not allowing me to be strong enough.

Tried to sell a House Wyvernspur retainer shapeshifted into a cow on the black market. The stupid part was I turned down 600 gold for him and then almost got arrested.

Back in early V3, there was a noble house called Fezznick, which had Helm as their Patron deity. The house was a close ally of the war wizards, the Purple Dragons, the City Watch. I was rather new to the server back in early 2004, and my language skills were not overly good. Anyhow, my CG halfling joined House Fezznick. As most of you know, Helm is big on all that vigilant business. VIGILANT! meaning being watchful, ever alert, good stuff for a guardian.
I thought it meant vigilantE! meaning, vengeance, taking the law into ones own hands, yada yada. Which meant my halfling, believing the best way to be a retainer, was to be the perfect helmite. Which meant I hunted down criminals, killed as many as I could, hunted the infamous Pyatt Pree, stuff like that. I dont really have to tell you all how that went down? I got kicked out of Fezznick, and died being executed for many counts of vigilantism by the guards. -

I was there for this one too. I played seter-elf's (Hound) right hand for awhile as a elf Bard/Fighter. He later got killed by said demon and her minions.

My dumbest moment was not killing Aldek when I had a split 2 seconds to do so.
I froze in utter shock and derped the attack.

in v2 when I stumbled into the slum invasion and found all these wonderful bags of gear laying about, and then turned around to see the side spawn of orcs with shamans laughing as they obliterated me with magic missles to where I had to scroll the log to see how many hit simultaneously sending my lvl3 so far into the fugue he left a nuclear shadow imprint in the snow

First character: walks into East Arabel and uses shout channel. "I NEED TWO TANKS AND TO BORROW 5000 COINS!" Banned shortly after.

A long time ago during V3 I was on a quest with a random group and we were fighting an undead boss. Well at least I thought he was undead. He looked very undead like to me. Our party was struggling and the only fighter in the mix of rogues and mages was close to downing the boss but just couldn't hit it for that last 1/4 of it's health. So I suddenly got a brilliant idea. Remembering that healing spells damage undead in table top I decided to cast an empowered cure serious wounds spell to finish it. The boss was fully healed. The party wiped completely. The worst part was I only had one spell left and that was my invisibility spell from the trickery domain. So I was also the only one to survive the debacle. Call it the evil machinations of Finder Wyvernspur. By the time they all made the walk of shame back to the quest area they were ready to lynch him. Especially since they now had a fully healed and ready to go boss to contend with in the next room.

I have been on the receiving end of this twice, actually. Though I don't remember which DMs anymore. I'm proud to say that my character survived one of them.

Can't even begin to tell you all the dumb things I have done on this server. So, since Hellball is mentioned, here is one about another DM to make Hominid feel better. Because I am tattling, I will not name the DM.
There was one DM that made a tool that had both the vFX and the real thing . Another kept clicking it wrong and managed to send Olen and the DM himself to the fugue. Happened more than once. Rare you see a DM damaged, rarer still to see one killed. Wish I got a screen shot of it.

In a massive PVP event i had protect evil activated instead of heal song.

Got to ask Zool, what's up with 1 pt negative? The 121 pts physical from a hit is not enough, but one little negative will do the job?

Actually, it's a staff a player was part way through making in late V4. It had +1 neg damage. I haven't been using my ban hammer (a great maul with a script that doesn't work, to ban people when I hit them) for some time now.
