Improve a Quest

I like optional sections and would love to see them on most quests. Particularly high level ones. They can take different forms.
Super boss (ala mmo raid: prismatic dragon)
fort defense (waves against grain)
endless waves (scaling reward with time spent fighting?)
optional dungeons (fey quest)
It could be cool to have stuff like this on most all quests, but require some special party member to gain access. For example, a warrior grants entry to the dungeon he's guarding only to paladins or something. A rogue has to detect the hidden passage and then unlock the doors to get in. The bard has to decipher the ancient language that will provide a password to enter. -
I'd like to see the already awesome Home Alone quest receive an optional boss after the optional area, perhaps at the hole at the very end.
Something like a named Drider Mage or something of the sort, a creature that is well and truly capable of ruining your day if you aren't prepared.
Of course the loot would have to scale to fit this boss. -

I mean, this post says you like what I've been doing to improve quests–-but just says "what you did is good". I want more unique/specific ideas that could keep me inspired to work through all our quests.

Nada: Malicious goblin druid attempting to breed an army of pests
Frin: Room filled with a maaaassive amount of dire rats
Tychean Shrine: if you go down the sewers, you are ambushed by a bunch of mid level undead.
Maysen's ring: Optional area near the shrine where you interrupt a cultist meeting -
Porry Hatter should have some ambiance, he's talking through a book why can't he say anything inside of the instance again? I thought the old rats were dead, why not have Porry's party animal arcane badgers that get drunk all the time fighting the hordes of an equally crappy wizard that happens to wield arcane rats in an epic battle.
The fire beetle path at the end looks like a good place for a bonus optional expansion quest where PC wizards might have an epic duel with the REAL WIZARD, achieve some minor item that is desired by sponsors, and have a challenging fight (based on the class of course).
