Uhm, it really depends which one you mean. You had Ty'rant, but he didn't really have any Tiamatan followers (He was largely part of meta-evil "Council of Supervillains" known as the Wheel).
He did eventually convert Salyndra, who would go on to lead another Tiamatan cult, but the group largely consisted of displaced elves (in character sheet only) from the current elven faction.
The second go was a bit more succesful (Among other things, eventually gaining a dragon patron), but still suffered form a fairly dubious standard of religon (The other main character in it was a half-orc who really played out their 7 int).

v1 to v2 I don't remember having anything IG. It was just a sudden vault wipe, though admittedly, I don't think I had even found the ezboard forums by then (which were used well into v2).
v2 to v3 was caused by the Xania, a PC paladin who died and returned to life when Cyric turned her into a blackguard (aka he became the DM AngrySquid), and set on a campaign the destroy the city, which happened.
v3 to v4 came when the narrative shifted with an outbreak of revolts against Suzail.
v4 to v5 comes when the Legion and Red Hart die peacefully in their sleep of old age, and their children carry on the legacy of sharing is caring.

Actually, the ultimate cause was a cataclysmic event triggered by the construction of Bolthor's shack. The whole Xania stuff was just fluff around it.

I remember logging Bolthor into fugue after the battles had basically killed everyone on the server (sans Moloch).

Yeah, v1 was just a wipe. v1 was prettymuch a public beta as it were, and sort of an experimental period as determing parts of the module.
v2 ended when Redshift's Thayan summoned a super-demon and its corresponding legions. Xania was just around. The event was rather tongue in cheek anyhow, as the demons included such things as a Gel Cube named "Crash Loss" and "LAG B3AST".
v3 was a new continuity entirely, and culminated at the Shadovar war.
v4 started off with the rebellion against Cormyr, and kicked off the Civil War thats still going on (albeit some of the sides have changed) now.
v4 mostly shares v3 continuity, though a few things took in retcons.
