I cannot give good information on this; I think it is found on the Boney Rat Sewer Goblin Quest.
It says 1 use, it has 3 charges.
It is set to Plot though, so you can use it infinitely.
I tossed the one I had in a bin.

@Mr-Moloch Also Tyrant Fog zombies spawn on this quest, I'm pretty sure you need Magic Weapon to kill them (they have DR 10 if I remember right). That makes them fairly unkillable for most non-caster classes.

The undead horses in this quest have their CR vastly wrong.
They're +7 AB and close to 50 HP, and spawn in groups of 3. When you kill them, they yield 8 XP on a level 5 character.
Compare to the skeleton hunter on this quest. +5 HP, around 20ish HP. They yield 36 xp.
My guess, based on experience, is that someone added a LOT of HP to these to make them tough to kill. But NWN doesn't recognize extra HP in the challenge rating. So you may have the CR artificially set too low or need to raise it higher.
That said, the undead skunk made my day.
