Fear

Some more add- ons, since I'm writting this as I get ready to go watch the jets kick some patriots ass and probably wrote my last post with less thought about all I want to say then usual-
You don't develop a scary reputation by snearing and acting evil. If you havn't done anything scary, why would anyone be scared of you? To take what maureen said, about vampires, tieflings, ect. If I really lived in arabel, I would either have a high immunity to fear considering all the threats it faces, or I would have moved. So yes, even your tiefling half lycan half dragon needs to do something scary to truly develop a reputation.

I don't want anyone to take this the wrong way, but I've seen very few PCs IG who truly rattle me. Yes, your blackguard in spiky armor with a glare that would make puppies shrivel up and die would, logically, be a good reason for my level X adventurer (especially if he's relatively unexperienced) to run like a rabbit from a pack of starved wolves, or simply piss himself on the spot. However, it's harder - at least for me - to RP fear if you're not really feeling it. Once a PC manages to make you think not just ICly, but OOCly "this guy's the Joker incarnate," fear just comes naturally.

I agree with Jagged.
For example: Most of my characters were shit-scared of Helmar Bloodchurn, even if only one of them saw him just once. He had a reputation that made people shiver and look behind them each time his name was mentioned. This really rubs off to others.
You don't need to be scary even, get some wingmen to spread tales about how crazy dangerous and scary you are and it just might stick.
And Helmar is a great example of a PC that gained IG fear but wasn't the "everyone fears me" type. He earned the fear in those he needed to scare but there were others who weren't afraid, respected him more then were scared, or were just as psycho and thought "hey this guy is like me!"

Was Darth Vader scary because he was a 7'2 guy in a suit or because he could crush people by just moving his hands or was he scary cause of the army of stormtroopers behind him?
Unless you have some OOC fear of the villain in front of you whether it a game a movie or a school yard bully it fails
Let go back to grade school what made your schoolyard bully (or you if you were one) Scary was it just your strength or size or was it because something else

Exactly. There is a difference between intimidated and scary. We all knew that guy who was super huge looked like he could kill the hulk who was a huge nice guy but when we first met him we were intimidated. Darth Vader looks intimidating. It's not til he uses his power/ shows why he should be feared that we become afraid.
