RP Advice: Insanity

I tried this for abit once in game talk to yourself talk to peaple that arn't there give peaple different names do stuipid things that will get you killed charge monsters with no armour barehanded as a spell caster(which in fact didn't get me killed but should of)Playing as two or more different peaple with one character is hard work so is the switching between them to make it fun.
Probably the hardest part will be doing things with the second personality that will get them killed as your wanting to keep them alive can get in the way. -
Insanity is nothing less than a state of confusion.
If you ask me, Hitler was insane, because he thought he could make a perfect world by slaughtering millions and millions of people, using whatever means necessary. He believed his atrocities appeased the original consciousness, and so he went on his ideas, like a madman.
So I consider anyone who acts out of a largely bloated ego (which is constituted by negative feelings like: hatred, spite, anger, envy, woe, fear, usury, greed, arrogance, pride, sadness, anxiety or depression) to be doing something crazy.
It also generally involves the person thinking they're someone they're not, or believing that they have capabilities which they haven't acquired yet. Like say: claiming to be the son of Tempus cough or an archmage.

Pick an altered reality to the current world and play in that instead.
By which i mean change your programmed reactions and alter the response that your character gives.
obvious one is good/evil, an erratic response or inconsistent works well but the key is that is not to say that you are evil, but that you dont know the difference…
so you might laud the necromancer one minute for his prowess with magic and frown on the paladin for stopping the game you are playing with the bodies, all the while having the thought that undead are an anathema.

It started off being a troll character, but could potentially be more fun.
He's mad, and has the intelligence and charisma to reflect that, but I'm having trouble portraying insanity without being over the top comedic. I'm looking for ways to reflect the aforementioned poor stats via lunacy without being a complete joke character. In fact, scary would be nice.
Maybe you could look up the DSM IV definitions and symptoms of various different mental disorders for ideas?
Stuff from fiction is always good - serial killers who manage to blend into society like Dexter, deranged yet beautiful maidens like Ophelia from Hamlet, borderline/narcissistic personality types like Livia Soprano from The Sopranos, high-functioning sociopaths like Sherlock Holmes in the new BBC version…
