"Accidental crash" might be one. Otherwise, you might want to hope that your teammates are not complete tossers and make the attempt to save you. Not likely, but its worth hoping for.

A week or so ago I was just out patrolling the wilds as Eagleman when I came across a group of Goblins, I took them all out fairly well, but the last one dazed me and then magic missled me down to like 5 to 10 HP or so and I was all sad I was going to die such a lame death, when thankfully I honest to goodness crashed and was saved! Best crash ever!
I logged back on right away (and risked dying) because I was worried the DM's might have thought I was being lame and purposely quit, but I had just enough time to transition and continue living!

Pray he only taps you to bleeding -1 then goes after someone else.
Some npc's will keep swinging at someone they've knocked down, but a fair number seem to find other people to beat up.

I would add to Lamancha's hypothesis the "no quest-friends around and no coin to pay for a raise" scenario. 40% xp loss guaranteed case.
Quest-friend, or quest-buddy: PCs you quest often with and trust to raise you back even when you may not have the coin to pay for it.
