            One i came up with with my DM in a past PnP campaign.

            A cursed weapon ( in the original case a great axe) infused with the essence of a demon. Cursed to jump into the hands of the one it considers to be its owner whenever said owner feels intense anger or feels threatened. This weapon can even jump across planes in order to do so. It can be passed on to a new owner, but ONLY if enough time passes for it to get used to the new owner. If it is passed to someone else, but the previous owner gets into a situation that causes it to be summoned before it is used to the new person, it will simply return.

            Other than a simple +1 enchantment and a small + fire damage, the item had limited sentience (spoke to its owners mind). As the campaign has progressed it has become more and more intelligent, to the point that it is likely to win an EGO check and take over the wielder before long.

            The Original was a simple +1 Greataxe with +1d6 fire damage.

            For NwN mechanics i would guess an item flagged as undropable. With instructions for when the Player MUST draw it. (and the odd weird voice in the head if a DM feels like some fun) and perhaps +1d4 fire damage, unless you think it deserves the big fiery glow of course.

              This parchment covered in red glowing runes is the lesser known cousin of the the more famous infernal contract. Instead of selling all of one's soul, the mortal who signs this document parts with a tiny portion of their soul in exchange for a temporary boon from the lower planes.

              Effect: Summon Monster 5(some sort of devil, caster level 12, user loses 100 experience)

                    These are items meant for characters ranging from 7-12 level wise. So a bit more powerful is ok.

                      The Armour of Grebus the Pit Fiend.

                      Created by Grebus when he achieved Pit Fiend Status. He gave it to a Blackguard name Boalath The Slayer as a gift. In return, Boalath was directed to build a castle in the forest near his village and dominate the area. This was rather unfortunate for the elves who occupied the forest. Boalath used it to great effect against the elves, removing it when needed and having an imp, provided by Grebus, carry it around for him. Eventually, Boalath's castle was surrounded in combat by a company of the elves who had sworn to take him down. Having stormed the castle, they fought Boalath for three whole hours on the battlements, without giving Boalath a single chance to rest or recover and remove his armour. Eventually Boalath's healing potions ran out, and the elves watched his life ebb away. When he was taking his final breaths, Grebus commanded his imp to take the body, creating a portal for it to do so. Rumours talk of screams echoing from that portal as Boalath cried out in final fury, knowing his life would be restored, but to one of eternal toment.

                      Base Item Full Plate:

                      The Armour of Grebus the Osyluth.

                      An earlier experiment of Grebus when he was still finding his form and capabilities. This one was given to a rogue named Boalath, who, wanting to gain power and strength, was willing to make certain sacrifices. He was a risk taker of extraordinary daring. Over time, Boalath became renouned in the land round his village for his blood thirst, especially for the elves who lived in the great forest to the North of his village.

                      Base Item Half Plate:

                      You have to carry it around, wearing other gear, only putting it on when you need the strength in combat. You then could die!

                          Said to the favoured weapon of serial subduesess who enjoyed making paladins fall from grace.

                          This whip constantly flicks about in combat like a viper and its snake like head has glowing red eyes and a nasty bit. It also often mutters words that it hears, but subtly twist them into a mocking manner.

                            This pepper was cursed with the souls of the damned, you can hear their resounding screams invade your ears when close enough.

                            Thrown Grenade like weapon. Blindness/deafness on hit.

                              Mirror to Hell

                              Item: A hand mirror, though a larger item which looks like a mirror you might need two holds to hand would be good too.

                              While priests may tell their followers of the horrors of hell, few living mortals can truly imagine the horrors of the damned. Jerenicus Wrathborn was a Hoaran crusader who devoted his life to punishing infernalists for their misdeeds. Yet Jerenicus did not think their deaths alone would be justice and so he crafted this mirror, into which he forced them to gaze. What dances across the surface is enough to drive any mortal nearly mad. When one gazes into this mirror, they see the brimstone and fire of the hells themselves and screaming souls crying out in tortured agony, clawing at the surface of the mirror, begging for a way out… Ironically, Jerenicus was hunted down and killed by a cabal of wizards who wished to study the hells. This mirror and replicas like it have since passed between the hands of those who wish to conjure the power of the hells and the forces of good, to teach their followers about the fate of the wicked...

                              Effect: One use per day, AOE Fear effect. High DC.

                              Every time the holder uses the mirror there is a chance they will catch a small chance glimpse of it, so also hit them with a fear effect, though a much lower DC, like 10.

                              Per the [[ ]] description, a DM could throw out some random fear effects on and near someone who has this in their inventory. Also a neat RP tool for a DM to have a random fiend or damned soul try to speak to the holder, through the mirror.

                                Heart of Bheleg

                                Chomel-al-Bheleg was a notorious pit fiend ravaging the lands of Mulhorand hundreds of years ago. Bound to the service of Setites, he has a habit of hunting down paladins of Horus-Re and enslave their souls into this large red gem. It is said that after years of torture in the hands of Bheleg, these souls became corrupted and twisted in infernal magics. Bheleg was finally hunted and slain by combined forces of Horus-Re and Osoris, but the gem was thought to be lost forever.

                                Upon concentrating to this fabled gem, the user is able to release one soul in the form of powerful spell, at the same time utterly annihilating the existence once noble paladin.

                                item: Magical Gem.
                                25 charges - as lvl 10 caster
                                5 charges / use - Bestow curse
                                5 charges / use - Fear
                                5 charges / use - Wall of fire
                                5 charges / use - Vampiric touch
                                5 charges / use - Negative energy burst

                                Babau talon Kama

                                Red slimy jelly-like acid drips from the blade of this weapon. This kama was crafted from a bone and talon of a Babau. While it gains some enchantments from natural abilities of said demon, only those of evil in nature are able to wield it.

                                Demon-skin coat

                                On closer inspection this horrible ragged cloak is actually a bloody skin of some demon. While it provides some useful protection through it´s magical abilities, it is utterly horrible looking and smelling, weakening the wielder and making pretty much everyone around the wielder feel uncomfortable.

                                Parrot of Abyss

                                This strange bird smells of rotten flesh and eggs. While not really sane person would tolerate it´s presence, this bird has an useful habit of throwing insults and rude-words at the foes of it´s "wielder" while sitting on their shoulder.

                                  The idea of this item is to create actual roleplayed weaknesses for evil characters.

                                  The skull of redemption

                                  Description when not worn :
                                  The amulet chain is made of an odd, silver metal. It holds what at first appears to be a shrunken head of a humanoid, likely a human being. The head seems to display unknown horror and pain inflicted, and the wearer of such an amulet must be truly a demented and sick individual.

                                  Base Item: Amulet

                                  Bonus: Intimidate + 6
                                  Bonus: Fireresistance: 5-
                                  Only usuable by: evil

                                  When put on, BAM, CURSED!

                                  The skull of redemtion: "The curse of Sir Jeofry"
                                  Description: As soon as you put on the amulet, you feel the chain snare itself around your neck. For several moments, it feels as if you are being strangled by the silvery chain. Just as you are about to collaps, you hear the roaring, slightly insane laughter from the skull as if awakens.

                                  Sir Jeofry was an ilmatery monk who for most of his life had worked to undo the deals made between mortals and devils. As he neared the end of his life, he encountered a young girl who had sold her soul to enhance her beauty and magical powers. No matter how much he tried to convince the girl to turn back upon the path of good, nothing could save her tormented being, fixated upon beauty.

                                  Knowing he was soon to die, he sought out a group of gypsies, and had a terrible curse placed upon himself, that would when he died, would place not only his soul, but his mind within a beautiful amulet he had purchased. Knowing the vanity of the woman, he showed her the amulet, and she killed him, triggering the curse.
                                  The amulet can only be taken off, when those who wear it have changed their ways.
                                  Through his teachings, the wearer gains a greater understanding of the world, while at the same time giving those without it the feeling of empathy and understanding of others.
